📋 Description

• Conduct remote monitoring teleconsultations focused on guidance, empathetic support, and self-care; • Provide continuous follow-up for participants, ensuring continuity of the care plan; • Identify clinical and behavioral needs and define personalized interventions; • Recognize signs of instability and refer the participant to appropriate care when necessary; • Refer to other members of the multidisciplinary team when indicated.

🎯 Requirements

• Bachelor's degree in Nursing • Postgraduate qualification in Public Health, Family Health, or related areas • Knowledge of chronic diseases, laboratory tests, and clinical guidance • Comfortable with digital tools, Microsoft Office, and navigating telehealth platforms • Experience with health programs and/or as a behavior change agent is a plus • Experience with remote work or telehealth consultations
