• Review clinical documentation to assign and sequence **ICD-10, CPT, and HCPCS** codes for various patient types (Hospital, Clinic, Ancillary, and Physician Pro Fee). • Analyze facility and provider records to ensure accurate **APC and Evaluation & Management (E/M)** coding. • Abstract clinical data and validate documentation adequacy to support diagnoses, procedures, and discharge dispositions. • Serve as a resource for providers and client staff regarding compliant coding practices and provider queries. • Maintain strict **HIPAA compliance** and uphold patient and provider confidentiality at all times. • Participate in team meetings, trainings, and continuing education sessions to stay current with coding updates and regulations. • Ensure consistent coding accuracy at or above the **95% quality threshold** while meeting productivity standards.
• Active **AHIMA** credential (RHIA, RHIT, CCS, or CCA) **or** **AAPC** credential (COC, CCS-P, CPC, or related). • Minimum of **2 years’ recent hands-on coding experience**, preferably in primary care or multi-specialty settings. • In-depth knowledge of **medical terminology, anatomy, physiology, pathophysiology, pharmacology**, and relevant coding systems (ICD-10, CPT, HCPCS). • Strong computer proficiency, including **MS Office (Outlook, Word, Excel)** and data entry accuracy. • Excellent communication, analytical, and problem-solving skills.
