• Assess and evaluate members' needs in a telephonic environment • Guide members/families toward resources for care and wellbeing • Employ strategies to manage members' health issues • Monitor patient care through assessments and evaluations • Create member care plans as needed • Follow established guidelines/procedures in decision making

🎯 Requirements

• Completed Bachelor's Degree in Nursing • 3 - 5 years of clinical acute care experience • Licensed Registered Nurse (RN) in the appropriate state with no disciplinary action • Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook • Must be passionate about contributing to an organization focused on continuously improving consumer experiences.
