• Candidate must live in Oklahoma • Works with members requiring intensive or complex care • Assess, facilitate, plan and coordinate integrated care • Develop and implement individualized care plans • Ensure member progress toward desired outcomes • Complete comprehensive assessments • Develop evidence-based, enrollee-centered interventions • Ensure timely access to quality care • Collaborate with medical and dental providers • Perform ongoing monitoring of care plans • Implement corrective actions as needed • Maintain ongoing member case load • Facilitate collaborative activities with Managed Care Organizations • Compile information and reports for evaluation • Improve workflows in case management program • Represent the company professionally • Complete special projects as requested
• Bachelor’s Degree in a health-related field • 3 years clinical experience or equivalent background • Preferred License, Certification: RN, LCSW, APSW, CCM, LPC/LPCC, RDH • 1-3 years in case management, disease management or managed care preferred • Well-organized • Exceptional organizational, written and oral communication skills • Knowledge of quality improvement methodologies and population health management preferred • Knowledge of healthcare advocacy activities is preferred • Knowledge of clinical and industry terminology and procedures • Self-motivated • Strong problem-solving skills
