📋 Description

• Support clients through the onboarding process using a positive, client-centred approach • Motivate clients to achieve health outcomes and ensure screening is carried out to high standards • Guide clients toward the most appropriate health intervention pathway • Use the case management system to accurately record referral information and timely case updates • Respond to information requests from clients, partner organisations, and the public • Collaborate with colleagues across Thrive Tribe services on new projects and service developments • Consider Information Governance and Clinical Governance legislation, requirements, and guidance • Help maintain Thrive Tribe’s organisational ethos and embed its values across services

🎯 Requirements

• Excellent customer service and communication skills • Empathetic and caring nature • Experience supporting clients to make healthy lifestyle choices is advantageous • Ability to offer flexibility regarding work pattern • Ability to work 30 hours per week, including Friday evening work until 8:00pm • Flexibility across daytime and evening hours • Possible Saturday working during peak periods • Right to work in the UK • CV and covering letter required for application
