📋 Description

• Deliver exceptional customer service while managing client referrals by phone and electronically • Accurately input client referral details into the database • Motivate service users to achieve health outcomes • Carry out screening to the highest standards in accordance with the NHS Code of Practice • Collaborate with the National Diabetes Prevention Programme team and partner organisations to create referral routes and meet targets • Prioritise key tasks according to service needs • Contribute to new projects and service developments across Thrive Tribe services • Comply with Information Governance and Clinical Governance legislation, requirements, and guidance • Help maintain and advance Thrive Tribe’s organisational culture and values

• Excellent customer service and communication skills • Empathetic and caring nature • Experience supporting clients to make healthy lifestyle choices is advantageous • Ability to work remotely from home • Right to work in the UK • Ability to attach a CV and covering letter when applying
