📋 Description
• Provides weekend oversight and clinical operations support for 4-6 branch locations in the state of Georgia. • Drives operational efficiency and supports branch-level field-based clinical teams. • Conducts/delegates the assessment and reassessment of patients, including updating care plans and interpreting patient needs. • Manages the assignment of caregivers and is responsible for the delivery of care to all patients served by the location. • Reviews available patient information related to the case to determine home health or hospice needs. • Assigns appropriate clinicians to a case and instructs them in delivering quality home care services. • Monitors cases to ensure documentation is compliant with regulatory agencies and requirements of third-party payers. • Coordinates communication between team members, attending physicians, and caregivers to ensure appropriateness of care and outcome planning. • Participates in sales and marketing initiatives.
🎯 Requirements
• Associate degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) strongly preferred. • Active and unrestricted Registered Nurse (RN) license or Compact State RN license with authorization to practice in the state of Georgia. • Proof of current CPR card and/or BLS certification. • A minimum of 2 years of experience as a Registered Nurse (RN). • Home health experience is required. • Management and people leadership experience is preferred. • OASIS experience is required. • OASIS certification (HCS-O, COQS, and/or COS-C), preferred. • Homecare Homebase (HCHB) experience is required. • Foundational knowledge and basic understanding of CMS PDGM is required.
