📋 Description
• Deliver primary care services via telemedicine • Provide high-quality, evidence-based medical care • Collaborate with a team to ensure continuity of care • Diagnose and manage a variety of adult/primary care conditions
🎯 Requirements
• Completed an accredited nursing or medical program with a national certification • Currently licensed in Illinois with the ability to obtain additional state licenses as needed • Valid Illinois DEA Registration in good standing with all schedules to prescribe medications, including S2 • At least 3 years post-residency practice experience is strongly preferred for APPs • At least 2 years post-residency practice experience is strongly preferred for Physicians • Must be Board Certified in Family or Internal Medicine • Commitment to evidence-based medicine • Willingness to learn how to diagnose and treat a variety of adult/primary care conditions • A PC with Windows PRO or a Mac newer than 2019 is required
