📋 Description
• Conduct telephonic (and occasional in-person) assessments of member health needs • Develop and implement individualized care plans to improve outcomes • Identify and address medical, behavioral, and social barriers to care • Coordinate care across programs, providers, and resources • Review claims/history to support effective case planning • Engage members using strong communication and motivational interviewing skills • Participate in case conferences and collaborate with clinical teams
🎯 Requirements
• Active, unrestricted multistate RN license (in state of residence) • Willingness to obtain additional state licenses (company-paid) • 3+ years of clinical RN experience • 1+ year experience using Microsoft Office and web-based systems • Case management experience in an integrated model (preferred) • Bilingual (Spanish/English) (preferred) • Strong technical/computer proficiency (preferred)
