📋 Description
• coordinating care for members facing chronic medical and behavioral health conditions • conducting comprehensive assessments • developing and implementing individualized care plans • documentation of assessments and interventions in member’s electronic health record • collaborating with the interdisciplinary care team • connecting members with health plan benefits and community resources
🎯 Requirements
• active and unrestricted Compact Registered Nurse (RN) licensure in the state of residence • proficient in Microsoft Office Suite, including Word, Excel, Outlook, OneNote, and Teams • 3+ years of nursing experience • 2+ years of case management, discharge planning and/or home healthcare coordination experience • bilingual skills, especially English-Spanish
