• Provide virtual consultations via DCA's digital platform • Conduct autonomous video, telephone and asynchronous consultations • Assess, diagnose and manage minor ailments and undifferentiated primary care conditions • Manage acute presentations including URTIs, UTIs, skin conditions, and ear, nose and throat conditions • Monitor long-term conditions, optimise medicines, and conduct complex polypharmacy reviews • Issue prescriptions independently in line with GPhC standards and DCA's prescribing governance framework • Review clinical information and prescribe asynchronously • Refer patients to GPs, ACPs, or specialist services when outside autonomous scope • Adhere to DCA clinical governance, CQC standards, and regulated activity requirements • Contribute to medicines safety reviews, significant event analyses, and audits • Maintain accurate clinical records in DCA's digital clinical systems • Support antibiotic stewardship and rational prescribing programmes • Participate in clinical supervision, peer review, and quality assurance • Collaborate with DCA's multidisciplinary clinical team and provide pharmacist expertise to clinicians • Contribute to service improvement, clinical pathway development, and digital innovation • Attend education and MDT events and engage in digital-health training • Promote and implement safeguarding protocols for adults, young people, children, and vulnerable adults • Implement requirements under the Mental Health Act 2007 and Mental Capacity Act 2005
• MPharm degree (or equivalent) with current registration with the General Pharmaceutical Council (GPhC) • Qualified Independent Prescriber (IP) with annotation recorded on the GPhC register • Minimum 1 year of independent prescribing experience • Must be able to prescribe autonomously from day one • Minimum 2 years' post-qualification experience working in a primary care setting • Completed the CPPE Care Pharmacy Education Pathway • Demonstrable experience autonomously managing minor ailments and undifferentiated primary care presentations without GP supervision • Experience conducting remote patient assessments by phone and video (desirable) • Proficiency with clinical IT systems, ideally Jira and Teams/Slack • Ability to work effectively under pressure and manage deadlines • Commitment to patient-centred care and ongoing professional development • Excellent written and verbal communication skills • Ability to document patient consultations according to organizational policy and national guidelines • Ability to recognize learning needs and pursue professional development in line with GPhC revalidation requirements • Ability to act as a professional role model • Skill in sharing knowledge and evidence-based insights • Ability to build relationships with internal and external stakeholders • Patient-centred approach • Current GPhC registration and prescribing annotation • Satisfactory DBS and reference checks
