📋 Description

• Work 1:1 with a consumer on the Medicaid waiver • Document notes within 24 hours of the end of a shift • Complete Annual In-Service Training including Respect and Dignity, Communication Training, Health Care Coordination, and more

🎯 Requirements

• Must be at least 18 years of age • Valid Drivers License - Learners Permit & State ID not accepted • Valid Auto Registration • Valid Auto Insurance Card • Driving Record • Social Security Card/Passport or Other Approved Identity Verification Document • Tuberculosis Screening (Negative - must be completed within the past 3 months) • CPR/First Aid Card (Front and Back)- Must Be An IN PERSON Certification • County Criminal History Record from each county of residence and or employment from the last 3 years • Background Check For Any State You Have Lived in the Past 3 Years • State (Limited) Criminal History Report - Indiana
