FORM is a virtual obesity medicine clinic that offers personalized, science-based weight loss programs. Patients have access to a Board-certified weight loss physician and a Registered Dietitian to develop a tailored plan that encompasses behavioral health, nutrition, physical activity, and FDA-approved medications, if appropriate. Utilizing the Form Health app, patients can engage in frequent video visits, message their Care Team, and monitor their progress. FORM partners with employers, health plans, and health providers to offer comprehensive obesity care. Their services are covered by national health insurance plans, including Medicare, focusing on a healthier future for their clients.

📋 Description

• Provide direct patient care in a clinical setting, focusing on evidence-based obesity treatment and chronic weight management. The physician will have their own panel and will collaborate with Registered Dietitians. • At this time, this role does not collaborate with APPs. In the future, this physician role will be responsible for chart reviews and conduct return visits for APP patients every third visit to ensure quality and continuity of care. • Provide treatment planning and medical management support for complex cases or escalated concerns. • Prescribe non-controlled obesity medications for patients under direct care • Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team. • Assist in the professional development of APPs by fostering clinical growth, offering feedback, and supporting continuing education. This role could include APP collaboration and supervision responsibilities. • Participate in team-based care initiatives and contribute to a culture of collaboration and high-quality care. • Attend relevant departmental meetings and contribute to quality improvement efforts within the obesity medicine program.

🎯 Requirements

• MD or DO degree with board certification; must be an American Board of Obesity Medicine (ABOM) diplomate or board certified in Endocrinology • Are licensed to practice and prescribe legally in GA. • LOQ (Letter of Qualification) with the IMLC (Interstate Medical Licensure Compact) is preferred. • Are or have in the past credentialed with insurance payers (strong preference for multiple state licenses!) • Have excellent interpersonal skills for patient and team interactions • Are committed to the highest quality of medical care and an evidence-based approach to obesity management • Have prior experience conducting telemedicine visits and who have a great "web-side" manner • Have prior experience working collaboratively with advanced practice professionals and registered dietitians • Bilingual candidates with medical fluency in Spanish are highly preferred
