📋 Description

• Act as Key Worker for a defined clinical caseload, ensuring continuity and coordination of care • Conduct holistic needs assessments and develop personalised care plans • Run nurse-led clinics and provide specialist breast oncology support • Deliver expert advice, education and psychological support to patients and families • Promote self-management and risk stratification approaches • Collaborate with consultants and the wider multidisciplinary team to ensure seamless pathways • Participate in MDT meetings and contribute to treatment planning • Lead or support quality improvement initiatives and service development • Contribute to audit, governance and clinical effectiveness monitoring • Maintain accurate and confidential patient documentation in line with regulatory standards • Develop, implement and evaluate a comprehensive specialist breast service • Support patients and their families throughout their cancer journey

🎯 Requirements

• Registered Adult Nurse with valid NMC registration • Post-registration qualification in cancer, palliative care or relevant speciality • Minimum 3 years’ relevant post-registration experience, with at least 2 years in cancer care • Experience in advanced communication and supporting patients with complex emotional needs • Evidence of leadership experience and multiprofessional working • Commitment to continuous professional development • Experience of audit, research or service development (desirable) • Teaching and assessing qualification (desirable)
