📋 Description

• Complete phone consultations with patients enrolled in care management and/or remote patient monitoring programs • Provide support and education about patients’ chronic conditions • Educate patients about health conditions and empower them with lifestyle and behavior strategies • Assist patients to set and reach goals aligned with provider-approved care plans • Maintain accurate and up-to-date patient records and document interactions and care plans per protocol • Address patient concerns and barriers to care and find practical solutions to improve adherence and outcomes • Communicate clearly, compassionately, and effectively with patients • Follow approved workflows for communicating patient needs to providers • Participate in training sessions, team meetings, and quality improvement initiatives • Respond to remotely transmitted patient data, including blood pressure, blood glucose, weight, and pulse oximetry, according to approved workflows • Establish relationships with assigned patients to empower chronic illness management and improve health

🎯 Requirements

• Current, valid, and in good standing Multistate/Compact Nursing License (LPN/LVN) required • Additional state licenses may be required • 3+ years of experience in primary care practice, cardiology, internal medicine, home care, or chronic care management/remote patient monitoring • Experience educating patients on chronic diseases such as hypertension and diabetes • Strong communication and interpersonal skills • Excellent organizational and time management abilities • Proficiency using electronic health records (EHR) and care management software • Ability to work independently and as part of a team • Empathy and a patient-centered approach to care • Reliable internet connection • HIPAA-compliant work area • Proficiency with virtual communication tools such as Zoom and Slack • Ability to work Monday through Friday, 9:00 a.m. to 5:30 p.m. Eastern Time, unless otherwise specified
