• Assess and evaluate members' needs and requirements in a telephonic environment • Achieve and maintain members' optimal wellness state • Guide members and families toward appropriate care and wellbeing resources • Facilitate interaction with relevant care resources • Manage members' physical, environmental, and psychosocial health issues using varied strategies and techniques • Identify and resolve barriers that hinder care • Monitor patient care through assessments and evaluations to ensure progress toward desired outcomes • Create member care plans when needed • Follow established guidelines and procedures • Report to the Associate Director, Health Services Nursing
🎯 Requirements
• 3 or more years of clinical acute care experience • Licensed Registered Nurse (RN) in the state of South Carolina with no disciplinary action • Comprehensive knowledge of Microsoft Office applications, including Word, Excel, and Outlook • South Carolina resident (preferred) • Bachelor's degree (preferred) • Experience with case management, discharge planning, and patient education for adult acute care (preferred) • Managed care experience (preferred) • Certified Case Manager (CCM) (preferred) • Minimum download speed of 25 Mbps and upload speed of 10 Mbps for home internet • Dedicated work space without ongoing interruptions to protect member PHI/HIPAA information
