📋 Description

• Provide comprehensive case management services to frail elders and adults with intellectual, developmental, or physical disabilities • Assess members' health and safety needs, strengths, interests, and preferences to develop comprehensive Member Care Plans • Coordinate with a Field Care Nurse to address members' health and safety needs in the least restrictive environment • Conduct face-to-face social assessments upon enrollment and at least every six months, typically at members' residences • Conduct quarterly in-person visits and maintain monthly phone contact • Arrange support services related to social integration, community resources, employment, housing, and other non-medical needs • Ensure cost-effective service delivery • Evaluate risk factors and educate members • Maintain accurate case notes, service authorizations, and Member Care Plan updates • Travel to client or assignment locations and occasionally to Humana offices for training or meetings

🎯 Requirements

• Bachelor's degree in human services or a related field and at least 1 year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities • Alternatively, a Bachelor's degree in another field and at least 3 years of such experience • Valid state driver's license • Proof of personal vehicle liability insurance with at least 100/300/100 limits • Reside within 45 minutes or less of the assigned coverage area is preferred • Case Management experience is preferred • Experience with electronic case note documentation is preferred • Knowledge of community health and social service agencies and additional community resources is preferred • Ability to travel up to 40% • Ability to work Monday–Friday, 8:00 am–4:30 pm CST • Minimum home internet speed of 25 Mbps download and 10 Mbps upload for home or hybrid home/office employees • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information • Required TB screening for this patient-facing role
