📋 Description

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury. • Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals. • Observe movements, conduct assessments, document progress and modify care plan as needed. • Educate patients on their plan of care, established goals, exercises and self-care. • Coach family on the plan of care, therapy goals and safely assisting with exercises. • Completes initial and ongoing comprehensive assessments at appropriate time points as assigned. • Provide interventions and therapeutic treatment in accordance with the physician certified plan of care and physical therapy professional standards of practice. • Regularly assess changes in the patient’s status since the last visit/treatment day, and determine whether the planned services should be modified. • Advise and consult with the family and/or other caregivers to promote the patient progress toward mutually agreed upon goals and planning for discharge.

🎯 Requirements

• Current license or certification as a physical therapist in the state of Virginia. • A graduate of an accredited physical therapy education program or equivalent as recognized by the American Physical Therapy Association (APTA). • In the last five (5) years, two (2) years of experience as a physical therapist that is relevant to the patient population assigned. • Covid-19 vaccination card • Valid driver’s license • Valid I-9 documentation • Current TB test or X-ray • Pass federal and state required criminal and abuse background checks where required.
