📋 Description

• Review referral information to identify priority, risk, and required OT intervention. • Carry out remote assessments via telephone and video call. • Provide preventative advice, guidance, and problem-solving support to adults, carers, and professionals. • Escalate safeguarding concerns in line with statutory procedures. • Apply strong functional questioning and clinical reasoning to assess needs remotely. • Prescribe standard equipment and minor adaptations in accordance with service guidelines. • Make defensible, evidence-based clinical decisions that reflect risk, strengths, and need. • Maintain accurate and timely case records. • Contribute to prevention-focused work, including equipment reviews, moving & handling advice, and self-management strategies. • Work collaboratively with community health, housing, social work, reablement, voluntary sector teams, and safeguarding partners. • Participate in supervision, MDT meetings, training, and service-development activities. • Promote a solution-focused and strengths-based culture.

🎯 Requirements

• Experience working in Adult Social Care, community OT, hospital discharge, reablement, or similar settings (Essential). • Experience managing complex cases and risk in fast-paced environments (Essential). • Experience completing remote assessments (Desirable).
