📋 Description
• Guide the design, launch, and expansion of consult services • Collaborate daily with physician specialists, executive leaders, NPs, PAs, and facility teams • Define clinical protocols, oversee quality metrics, mentor providers, manage multi-site operations • Monitor dashboards, ensure CMS and state compliance • Lead RCAs when needed, and act as the clinical connector aligning strategy, quality, workforce development, and operational execution
🎯 Requirements
• Master of Science in Nursing (MSN) • Active Nurse Practitioner license in good standing (multi-state preferred) • 10+ years of NP experience in PALTC, SNF, or LTACH settings • 5+ years of experience in leadership, education, or program development • Clinical background in cardiology, pulmonology, nephrology, or infectious disease • Proven success building and leading NPP teams • Strong knowledge of CMS regulations, documentation standards, and value-based care • Proficiency with EHR systems, dashboards, and care coordination platforms
