📋 Description

• Develop and manage standardized discharge and long-term recovery plans, including goal-setting, swipe cadence, and follow-up checkpoints • Engage directly with patients via live calls and digital tools to reinforce therapy understanding, adherence, and motivation • Administer surveys and collect long-term outcomes data to evaluate functional progress, satisfaction, and adherence trends • Document engagement activities and patient progress in Salesforce, flagging at-risk patients for clinical escalation • As needed, support promotion or execution of patient engagement events, both virtual and in-person, to foster community and encourage sustained use • Collaborate with stroke coordinators, therapists, and physicians to ensure continuity of care and seamless communication • Coordinate with physicians or therapy sites for milestone visits, including battery checks and device management follow-ups • Ensure all patient communications and data handling comply with consent, privacy, and regulatory requirements • Capture patient stories and feedback to illustrate success, improve engagement, and inform long-term program enhancements • Interface with digital engagement tools that automate outreach and improve patient connectivity

🎯 Requirements

• Customer service experience a plus • Licensed Occupational Therapist (OT) or equivalent neurorehab clinician is a plus • 2+ years of neurorehabilitation experience, including stroke recovery and patient education (preferred) • Experience in a consultative role or customer service roles are a plus • Familiarity with CRM systems (Salesforce) and outcomes tracking tools • Ability to collaborate across disciplines — therapists, physicians, sales, and patients • Understanding of stroke recovery and neurorehabilitation care pathways • Strong written, verbal, and interpersonal communication skills • Skilled at patient motivation, expectation-setting, and coaching • Comfortable with virtual patient engagement from a home office environment • Proficient with Microsoft Office Suite and CRM systems (Salesforce)
