📋 Description

• Engage members facing mental health and/or substance utilization disorders and build supportive relationships • Conduct holistic health assessments and identify social determinants of health and cultural influences • Collaborate with members, caregivers, and providers to develop member-centric case management plans • Monitor, evaluate, and update case management plans and progress toward goals • Facilitate communication and care coordination among the healthcare team • Provide interventions to remove barriers and promote positive behavior change using motivational interviewing • Stabilize members' natural support systems by identifying and addressing caregiver needs • Maintain concise, accurate, and timely documentation meeting accreditation, contractual, and legal requirements

🎯 Requirements

• Active WA State or Compact license allowing independent assessments; accepted behavioral health credentials include LMFT, LICSW, LMHC, and LPC • Other CCMC-eligible healthcare licensure • 3 years' post-licensure direct care experience • 1 year of post-licensure case management experience involving care coordination, care planning, discharge planning, or management of patient care over time • CCM certification preferred; if not held at hire, must be obtained within 24 months of hire • 4 or more years of direct care experience across multiple patient populations, conditions, or care settings preferred • Motivational interviewing experience preferred • Chronic Condition Certifications preferred • Integrated Case Management certificate (ICM) preferred • Chemical Dependency Professional (CDP) certification preferred • LEAN training preferred • Individuals residing outside Washington must have or obtain the appropriate Washington State license and apply within 10 days of acceptance • Internet access through any ISP with a minimum download speed of 75 Mbps • Ability to work the stated Pacific Time training and ongoing coverage schedules
