📋 Description
• Serve as the primary contact for patients on the RPM365 platform. • Assist with device setup and app connectivity. • Engage with patients to ensure adherence to care plans. • Schedule monthly (or more) check-ins and monitor patient data. • Educate patients through shared resources. • Notify healthcare providers of critical alerts. • Attend virtual team meetings for updates.
🎯 Requirements
• Active New York State nursing license • Knowledge of chronic conditions and related monitoring devices • Proficiency with technology, computer platforms, and smartphones • Strong verbal/written communication skills • Comfortable with patient outreach and engagement • Highly organized, detail-oriented, and team-focused • Independently motivated
