• Assist the clinic care team in managing and coordinating preventative screenings and wellness strategies • Monitor provider patient attribution and contribute to electronic medical record integrity • Coordinate wellness screenings for specific specialties • Support office visit, procedure, and scheduling screenings • Manage chronic disease registries, provider scorecards, and result reconciliation • Promote quality initiatives and improved patient outcomes • Participate in preventative care and health maintenance for age-related patients • Track and report quality measures and validate patient record accuracy and integrity • Collect subjective and objective patient or caregiver data to identify potential barriers to care and refer to care management roles • Communicate collected data and obtain appropriate orders to support the healthcare team • Follow through on patients’ plans of care under the direction of the care team • Make outreach to patients and healthcare providers to coordinate care and connect patients with services, care team members, and resources
• Certified Medical Assistant (CMA) background is required • Post-secondary education of one to two years with an emphasis in healthcare or a medical-related field is preferred • Minimum of one year of work experience in ambulatory care, community outreach, patient or provider support, customer service, and/or integrated care systems is preferred • Training in Motivational Interviewing and/or Mental Health First Aid is preferred • Computer literacy, including Microsoft Office products and electronic health records (EHR) • High level of computer skills and ability to run registries • Customer service, critical thinking, and multitasking skills • Knowledge of medical and medication terminology • Knowledge of medical paperwork, including insurance completion/submission requirements • Ability to function within administration pre-defined scope-of-practice guidelines
