📋 Description

• Carry out reviews for patients with long-term conditions within clinical competencies • Provide specialist pharmaceutical support to patients supported via NHS Partners • Manage chronic diseases, repeat prescriptions, and structured medication reviews • Collect and record patients’ health data holistically • Interpret, risk-stratify, and manage data according to standard operating procedures and NICE guidelines • Provide evidence-based lifestyle advice • Prescribe or alter medications for patients with long-term conditions • Maintain patient safety and dignity • Build and maintain trust with patients • Maintain contemporaneous clinical notes in Suvera’s and GP Partners’ systems • Participate proactively in the Quality Outcomes Framework (QOF) • Undertake other reasonably required administrative duties • Work across multiple partner organisations in a predominantly patient-facing role • Collaborate with the wider clinical team and report to a Clinical Team Lead

🎯 Requirements

• Minimum 12 months’ experience as a clinical pharmacist within an NHS Primary Care setting • Current membership of a medical professional defense organisation • Independent prescriber status • Completed a GPhC-accredited course or equivalent • Currently registered with the GPC and compliant with its Code of Professional Conduct • Excellent written and spoken communication skills • Confidence communicating with patients via email, messaging, telephone, or video calls • IT literacy and ability to use systems including EMIS Web, SystmOne, and Microsoft Office • Confidence managing hypertension, Type 2 diabetes, asthma, lipids, structured medication reviews, frailty, CKD, heart failure, and COPD • Completed, or made progress toward completing, the CPPE Pathway • Enthusiasm for improving patients’ health and wellbeing • Interest in transforming healthcare delivery for patients with long-term conditions • Personable, polite, and approachable manner • Flexible, proactive, and willing to learn • Team-oriented approach • Awareness of personal limitations when delivering care • Comfort seeking advice and support from the wider clinical team when necessary • Previous digital healthcare provider experience is a nice-to-have
