• Providing proactive intervention and coordination of care to eligible Vaya Health members. • Works with the member and care team to identify and alleviate inappropriate levels of care. • Supports and may provide clinical transition planning assistance to state and community hospitals and residential facilities. • Conducting comprehensive bio-psycho-social assessments addressing health-related resource needs. • Participation in interdisciplinary care team meetings, and ongoing care management. • Managing Child/Adult High-Risk Team meetings in collaboration with community stakeholders.
• Master’s degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area. • 2 years of experience working directly with individuals with BH conditions • 2 years of prior Long-tern Services and Supports and/or Home Community Based Services coordination, care delivery monitoring and care management experience. • Valid licensure required: RN, LCSW, LCSWA, LCMHC, LCMHCA, LCMHCS, LPA, HSP-PA, LCAS, LCASA, LMFT, LMFTA. • Knowledge of the NC Medicaid program, NC Medicaid Transformation, Tailored Plans, and state-funded services is preferred.
