📋 Description

• Serve as the primary coordinator for clients with complex needs • Partner with healthcare providers, specialists, pharmacists, social service agencies, caregivers, family support persons, and other care-team stakeholders • Manage client cases and coordinate healthcare benefits • Provide education and facilitate timely, cost-effective access to care • Promote wellness, recovery, independence, resilience, and member empowerment • Assess member needs across physical health, mental health, substance use disorder, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and community services • Oversee development of client care plans and goals • Offer office-based, telehealth, or field-based services where members reside, seek care, or can access services most easily • Connect clients with needed social services and supports • Advocate for clients with healthcare professionals • Use Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles • Conduct outreach and engagement activities and document activity in the CRM system • Evaluate client progress and update SMART goals • Provide mental health promotion • Arrange transportation, such as ACCESS • Complete documentation and outcome measures within care-plan timeframes • Maintain current patient health records in the EMR and other business systems • Complete monthly reporting for program compliance • Attend assigned training

🎯 Requirements

• 2+ years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations • Willing and able to work Monday-Friday, 8:30am-5:00pm Pacific Time, with flexibility for potential evenings and weekends • Working knowledge of government and community resources related to social determinants of health • Clean driving record, valid driver's license, and reliable transportation • Excellent oral and written communication skills • Positive interpersonal skills • General computer skills and working knowledge of Google Workspace, MS Office, and the internet • Bilingual English/Spanish preferred • Applicants must reside in Arizona, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, Utah, or Washington • Background check required prior to employment, potentially including drug test or other health screenings • Employees in patient, client, or customer-facing roles must be vaccinated against influenza, subject to accommodation consideration • Must complete identity and work eligibility verification using E-Verify upon hire • Occasional travel may be required • Ability to perform regular sitting, standing, walking, talking, listening, observation, computer work, and laptop/video-conferencing use; occasional lifting of items weighing 25 pounds or less
