• Provide individualized support to patients via in-person meetings, phone, email, and online platforms. • Educate patients about prostate cancer, treatment options, and available resources. • Complete distress screening with patients to determine their current needs, triage patients to appropriate resources and ensure follow-up. • Provide emotional support to patients, leading with empathy and compassion. • Lead patient and/or caregiver support groups. • Maintain accurate documentation of all patient interactions. • Identify trends and share insights with the navigation team to inform programming. • Other duties as assigned
• 3 – 5 years of experience in patient navigation, health education, or social services. • Knowledge of healthcare systems, insurance, and community resources. • Experience in patient education, care coordination, or guiding individuals/families through complex healthcare systems. • Strong documentation and data entry skills, with experience using CRM or case management systems. • Ability to manage multiple cases effectively. • Excellent communication and interpersonal skills, with the ability to build trust and rapport with diverse populations.
