• Stabilize high-risk IKC patients through intensive care coordination to reduce hospital readmissions • Support the high care pod team • Coordinate and facilitate quality, cost-effective care while minimizing fragmentation for ESRD and CKD patients • Coordinate care among patients, families, physicians, providers, care teams, the community, and the Regional Operations Manager • Manage clinical, financial, and patient satisfaction outcomes • Apply change-agent practices and CQI techniques • Provide healthcare performance coaching • Adapt practice patterns to new electronic processes and productivity expectations related to Capella implementation • Use DaVita clinical software, including Capella • Collaborate autonomously and with the healthcare team to deliver quality patient care
• A.D.N. degree from an accredited school of nursing required; a three-year diploma from an accredited diploma program may be substituted • B.S.N. preferred • Current Registered Nurse (RN) license in the state of practice required • Continuing education credits maintained as required by state of practice required • Minimum of five (5) years’ experience in clinical nursing required • Minimum of three (3) years’ experience in renal nursing preferred • Demonstrated knowledge and understanding of data and management of clinical, financial, and patient satisfaction outcomes • Demonstrated experience and effectiveness in a change-agent role • Demonstrated knowledge and understanding of CQI techniques • Previous experience in healthcare performance coaching required • Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred • Current CPR certification required • Ability to adapt to new electronic processes and increased productivity expectations related to Capella implementation • Current driver’s license in the state employed, positive driving record, and ability to meet insurance coverage requirements required • Basic computer skills and proficiency in MS Word and Outlook required • Functional proficiency with DaVita-specific clinical software programs, including Capella, required within 90 days of employment • Home office with internet connectivity of at least 1MB upload and 1MB download speed required
