Descrição

• Telephonically assess, evaluate, and support members with mental illness and complex medical situations • Guide members and families toward appropriate care and wellbeing resources • Educate and empower members to make healthcare decisions • Provide guidance and clinical expertise for navigating the healthcare setting • Conduct ongoing telephone consultations to support quality care • Collaborate with a multidisciplinary team • Manage members’ physical, environmental, and psychosocial health issues • Identify and resolve barriers to effective care • Monitor progress toward desired outcomes through assessments and evaluations • Create member care plans as needed • Coordinate community care and services • Conduct proactive telephonic outreach to eligible Humana members • Complete physical, psychological, emotional, and environmental assessments • Recommend and coordinate interventions, including information, education, resources, and referrals • Work with Interdisciplinary team members including LAL, IHWA, Social Services, Pharmacists, Psychiatrists, Utilization Management teams, Community Health Educators, and EAP • Use Motivational Interviewing and Solution-Oriented approaches • Comply with Humana performance and reporting standards

🎯 Requisitos

• Bachelor's degree • 3+ years of experience working as a Licensed Clinical Social Worker or Licensed Clinical Professional Counselor • Licensure in field of study within the state of Illinois with no disciplinary action, or ability to obtain Illinois licensure • 1 year of managed care experience • Passion for contributing to an organization focused on improving consumer experiences • Preferred: Master's degree in a behavioral-health field from an accredited university • Preferred: Bilingual proficiency in English/Spanish, English/Polish, or English/Creole, including speaking, reading, and writing without limitations or assistance • Preferred: Certified Case Manager (CCM) • Preferred: Experience with case management, discharge planning, and patient education for adult acute care • Preferred: Managed care experience • Preferred: Prior experience with Medicare and Medicaid recipients • Preferred: Experience with electronic case note documentation and multiple computer applications/systems • Preferred: Experience with health promotion, coaching, and wellness • Preferred: Knowledge of community health and social service agencies and community resources • Must meet language proficiency testing requirements when speaking with members in a language other than English • Home internet service with minimum 25 Mbps download and 10 Mbps upload speeds • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
