• Coordinate and oversee behavioral health services to ensure appropriate, high-quality care • Conduct holistic assessments and identify barriers to care • Develop and adjust individualized care plans to support recovery and self-management • Coordinate behavioral health services, care transitions, crisis prevention plans, and aftercare • Partner with members, providers, and health plan medical management teams • Build trusting relationships with members and providers using motivational interviewing, education, and ongoing communication • Serve as the single point of contact for members • Facilitate information exchange, discharge planning, and connections to behavioral health, community, and support resources • Ensure accurate and timely documentation and service authorization • Participate in audits, quality improvement initiatives, case presentations, and professional development • Support compliance with medical necessity criteria, regulatory requirements, accreditation standards, and internal policies
• Current, unrestricted state license to independently practice as a Clinical Social Worker, Marriage and Family Therapist, Professional Counselor, Clinical Psychologist, or Registered Nurse in the applicable state(s) • 3-5 years of direct clinical care experience in facility-based and/or outpatient behavioral health or substance use treatment settings • Strong clinical judgment and case management skills • Ability to support members through complex care transitions • Excellent verbal and written communication skills • Proficiency in Microsoft Outlook and Word • Ability to document in real time while engaging with members • Ability to interpret and apply medical policy and clinical criteria • Ability to pass background checks upon hire and throughout employment, including criminal, identity, education, employment, sanctions, watchlist, sex offender registry, and 10-panel drug screening • High-speed hardwired internet service with minimum download speed of 20 Mbps, upload speed of 5 Mbps, and maximum latency of 100 milliseconds • Ability to work independently to meet caseload requirements and team objectives • Interest in professional development through specialization, certification, and/or advanced degree • Quiet workspace with minimal background noise • Ability to work from a fixed, designated workspace in the home • Ability to sit for extended periods and use company-issued wired headsets and desktop equipment
