Descrição

• Provide complex care coordination and monitoring to members with primary Intellectual or Developmental Disabilities and possible secondary mental health, substance abuse, or complex medical/physical health needs • Use person-centered planning methods and strategies to gather information and address individual member needs • Provide education and support regarding members' rights and responsibilities • Complete discovery activities, information gathering, and assessments as part of the planning process • Ensure assessments and plans are updated when needed and when members' life circumstances change • Support integrated care management through multidisciplinary collaboration • Monitor quality support and adherence to waiver requirements • Travel within Trillium's catchment as required

🎯 Requisitos

• Must meet QP status • Fully licensed by the NC governing board regulating Human Services professions; OR fully licensed RN licensed to practice in North Carolina by the NC Board of Nursing with four (4) years of full-time experience with the MH/DD/SA population served; OR master's degree in a human service field with one year of full-time pre- or post-graduate degree MH/DD/SA experience; OR bachelor's degree in a human service field with two years of full-time pre- or post-bachelor's degree MH/DD/SA experience; OR bachelor's degree in another field with four years of full-time pre- or post-bachelor's degree MH/DD/SA experience • Substance abuse professional alternatives require the specified supervised alcoholism and drug abuse counseling experience • For members with LTSS needs: minimum two (2) years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience; may be concurrent with QP experience • Valid driver's license • Must reside within 75 miles of Trillium's North Carolina catchment • Must be able to travel within the catchment as required • Resume must include employer names, dates of service, average hours worked per week, essential job duties, education details, and licensure/certification information • Must pass a drug test as a condition of employment
