Beschreibung

• Develop and implement individual crisis and safety plans with youth and families • Facilitate referrals and linkages, monitor care plan implementation, and manage 1915(c) Waiver funds • Obtain prior authorization and Ohio Department of Medicaid approval • Conduct outreach, assessments, meetings, and care coordination in person, in the community, or virtually according to guardian/member preference • Manage Intensive Care Coordination and Moderate Care Coordination caseloads • Integrate comprehensive and evidence-based strategies into Child and Family Team meetings • Engage newly referred youth and families in care coordination • Develop Child and Family-Centered Care Plans and identify client family teams • Facilitate monthly or more frequent Child and Family Team meetings • Complete comprehensive assessments, including Child and Adolescent Needs and Strength assessments • Apply trauma-informed care, culturally responsive approaches, and the wraparound model • Coach and develop new staff • Coordinate care with providers, agencies, physical health providers, and school personnel • Monitor team commitments and CFCCP implementation and communicate progress to team members • Develop community resources to meet youth and family needs • Complete timely and accurate care plan documentation and bill for care coordination activities • Document team attendance, approvals, signed CFCCPs, SMART goals, and authorization processes in the electronic health record • Consult on documentation standards and electronic health record improvements

🎯 Anforderungen

• Scioto/Lawrence County residents only • Bachelor’s degree and 2+ years’ experience and training in one or more of: Family systems; Community systems and resources; Case management; Child and Family counseling or therapy; Child protection; Child development • OR Master’s degree with 1 year experience in one or more of the listed areas • May require clinical license, certification or training
