Beschreibung
• Conduct housing assessments and determine client eligibility for housing caseload of 20 clients per month in person. • Assist clients with housing search, applications, and move-in coordination • Build relationships with landlords and local housing programs • Support clients in maintaining lease compliance and resolving tenancy issues • Provide ongoing tenancy-sustaining services (budgeting, life skills, advocacy) • Collaborate with MCO housing teams and Grace Community Care’s intake and billing departments • Maintain accurate documentation and case notes in Microsoft. • Participate in regular team meetings and training
🎯 Anforderungen
• Experience in case management and healthcare required. • Knowledge of NJ familycare/medicaid, DDD, or Section 8 preferred • Strong communication and organizational skills • Valid NJ driver’s license and reliable transportation • Ability to travel 100% locally for home visits or landlord meetings. • Must have your own computer and highspeed internet. • Ability to meet with clients at a safe library. • 100% field visits required.
