Beschreibung

• Assess and evaluate members' needs and requirements to achieve or maintain optimal wellness • Guide members and families toward appropriate care and wellbeing resources • Facilitate interaction with community and healthcare resources • Manage members' physical, environmental, and psychosocial health issues using varied strategies and techniques • Identify and resolve barriers to effective care • Monitor patient care through ongoing assessments and evaluations • Ensure patients progress toward desired outcomes • Create member care plans when needed • Interpret guidelines and independently determine appropriate courses of action • Make decisions regarding work methods in ambiguous situations with minimal direction • Follow established guidelines and procedures

🎯 Anforderungen

• Registered Nurse with 2 years of experience in in-home case/care management • Experience working with the adult population • Knowledge of community health and social service agencies and additional community resources • Ability to travel to members' residences within 30 to 40 miles • Exceptional communication and interpersonal skills with ability to quickly build rapport • Ability to work with minimal supervision within the role and scope • Ability to use electronic information applications/software programs, including electronic medical records • Intermediate to Advanced computer skills and experience with Microsoft Word, Outlook, and Excel • Excellent keyboard and web navigation skills • Ability to work a full-time, 40-hours-minimum flexible work schedule • Valid state driver's license • Personal vehicle liability insurance meeting state minimums or specified higher limits • Valid driver's license, car insurance, and access to an automobile • Separate room with a locked door usable as a home office for continuous privacy • Access to high-speed DSL or cable modem; satellite internet service is not allowed • Required TB screening for this patient-facing role • Passion for continuously improving consumer experiences • BSN preferred • 3–5 years of in-home assessment and care coordination experience preferred • Experience with health promotion, coaching, and wellness preferred • Previous managed care experience preferred • Bilingual English and Spanish preferred • Certification in Case Management preferred • Motivational Interviewing Certification and/or knowledge preferred
