Beschreibung

• Travel to members’ homes and community settings to deliver episodic, longitudinal, and urgent pediatric care. • Build trusted, compassionate relationships with children and families, ensuring coordinated, culturally responsive care. • Collaborate within an integrated care team including physicians, APPs, nurses, pharmacists, dietitians, navigators, social workers, and therapists. • Conduct and document in-home assessments; perform procedures such as ear irrigation, point-of-care testing, IV placement and lab draws, wound care, and medication administration. • Provide patient and family education, follow-up care, and care coordination using the Imagine Pediatrics platform. • Flex into the virtual acute care RN role to conduct virtual assessments and manage urgent pediatric needs using established protocols. • Document care in the EMR (Athena) and communicate with families through Imagine Pediatrics’ proprietary application. • Utilize virtual platforms and digital tools confidently to support care delivery. • Perform other duties as assigned.

🎯 Anforderungen

• Active RN compact license required. • PALS & BLS required • Bachelor's degree in nursing (BSN) is required. • EMS (EMT/P) experience and 2+ years of pediatric home care experience preferred • Bilingual Spanish is preferred • Minimum 3+ years of pediatric nursing experience, ideally including high-acuity settings (ED, urgent care, transport, or similar). • 1–2 years in pediatric emergency medicine experience. • Valid driver's license, reliable transportation, and good driving record. • Comfort driving long distances in varied weather conditions. • Strong clinical judgment, communication skills, and ability to work independently in the home environment. • Comfort with EMRs, virtual platforms, and mobile documentation tools.
