Beschreibung

• Provide compassionate, client-centered personal care and supportive services to clients in their homes • Assist with activities of daily living, including bathing, dressing, grooming, toileting, ambulation, and feeding • Assist with prescribed exercises and mobility as trained and permitted by regulation • Provide medication reminders as directed by the nursing supervisor • Take and record vital signs and report changes in client condition • Prepare meals and assist with feeding as needed • Perform light housekeeping to support a safe and clean home environment • Provide companionship and emotional support • Document care provided and report changes or concerns promptly • Use GPS/EVV app and company software for clocking in and out • Communicate with the Scheduling Coordinator regarding assignments, availability, and scheduling needs • Follow individualized care plans as directed by nursing staff • Adhere to agency policies, procedures, and applicable state regulations • Maintain client confidentiality in accordance with HIPAA guidelines • Attend required training sessions and in-service education • Maintain professional boundaries and uphold the agency’s code of conduct • Work under the supervision of the Nursing Supervisor and direction of the Scheduling Coordinator

🎯 Anforderungen

• Current Home Health Aide certification or Certified Nursing Assistant license in Massachusetts • CPR certification (current and valid) • Excellent interpersonal skills and ability to work independently • Reliable transportation and willingness to travel within the service area • Ability to stand, walk, bend, and lift for extended periods • May be required to bend, stoop, reach and move client weight up to 250 pounds • Ability to lift/carry up to 30 pounds • Flexible availability including weekdays, weekends, evenings, and holidays depending on client needs and agency requirements • High School Diploma or equivalent (preferred) • One (1) year of experience in a home care setting preferred
