Beschreibung

• Review clinical records for clinical documentation integrity and mortality scoring • Collaborate with others on clinical documentation improvement and risk-adjustment findings • Provide feedback to clinical staff on CMS clinical documentation requirements during concurrent record review • Serve as liaison between coding staff and physicians • Identify opportunities for new program development, program extensions, and process improvement based on outcomes analysis • Work with team members to improve standard work • Maintain documentation and logs of adjusted risk and CDI opportunities • Apply evidence-based practice and scientific knowledge to nursing practice and age-specific and clinical patient needs • Perform other assigned duties

🎯 Anforderungen

• Graduate of an accredited school of nursing, PA, NP, or medical school, OR Associate's degree and Certified Clinical Documentation Specialist (CCDS) certification from ACDIS • Two years in an acute care setting with two years of experience in clinical documentation, OR four years of experience in clinical documentation with CCDS certification • Required professional license and/or certification appropriate to the state of work location: CCDS, physician assistant license, physician credentialing, RN, APN/APRN, nurse practitioner, or applicable equivalent • Knowledge of CMS requirements related to clinical documentation • Knowledge of mortality models, O/E ratios, industry trends, variable and DRG frequency • Frequent keyboard use/data entry • Ability to perform required physical activities, including frequent lifting/carrying and pushing/pulling objects weighing 0–25 lbs and occasional lifting/carrying and pushing/pulling objects weighing 25–50 lbs • Ability to work in a constant state of alertness and safe manner
