Beschreibung

• Manage ongoing care for assigned individuals to support continuity and stability across services and settings • Coordinate services across providers and community resources for long-term care management in a facility setting • Provide education on services and community resources • Link individuals to community resources and services for community-based integration • Engage individuals and families to support sustained participation in services and care plans • Serve as a member of the My Choice program for IDD members living in an intermediate care facility and/or State Developmental Center • Assess needs, develop care plans, and coordinate services to promote continuity of care and long-term outcomes

🎯 Anforderungen

• Must meet QP status through one of the specified pathways: qualifying NC human-services professional license; fully licensed RN licensed to practice in North Carolina plus four years of full-time MH/DD/SA experience; master's degree in a human service field plus one year of relevant experience; bachelor's degree in a human service field plus two years of relevant experience; or bachelor's degree in another field plus four years of relevant experience • Substance abuse professionals must have the specified supervised alcoholism and drug abuse counseling experience • For members with LTSS needs, minimum two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience • Fully vaccinated against Covid-19, Influenza, MMR, Varicella, and Tdap, with required proof or immunity documentation • Must have a valid driver's license • Must reside within Trillium's Mid State or Southern region, specifically Anson, Richmond, Moore, Scotland, Hoke, or Robeson counties • Must be able to travel within catchment as required • All candidates must pass a drug test as a condition of employment • Resume must include employer names, dates of service, average weekly hours, essential duties, education details, and licensure/certification information
