Beschreibung

• Outreach to referred and eligible patients to introduce care management services, obtain verbal enrollment consent, and document in the EHR • Make outgoing calls to help patients manage chronic diseases through condition and treatment education, medication management, and appointment management • Coordinate care across the continuum for patients with chronic cardiovascular conditions with cardiologists, advanced practice providers, and multidisciplinary teams • Assess patient health and social determinants of health needs and document and implement care plans • Serve as a liaison for interdisciplinary teams and community organizations across inpatient and outpatient care organizations • Provide patient- and family-centered care in outpatient and telehealth settings, including triaging calls and EHR messages • Educate patients and caregivers regarding health conditions, care, transitions of care, and chronic care management • Evaluate remote patient monitoring technology, diagnostics, and laboratory results and engage advanced practice providers for appropriate interventions • Ensure CMS documentation and billing guidelines are followed in the EHR

🎯 Anforderungen

• Excellent clinical, communication, and organizational skills • Ability to multitask and perform critical analysis • Ability to function independently within the limits of the position • Ability to function in a team environment • Knowledge and competence in care management, including transitional care management and complex care management • Knowledge and competence in basic cardiovascular technology, including EKG, patient assessment, care management, labs, and hospital test results • Strong understanding of population health concepts and clinical documentation requirements • Interpersonal skills to be approachable and understandable to patients, families, and care team members • Minimum associate's degree in nursing and graduation from an accredited nursing program • Current registered nurse license with a compact license or licensure for Alabama, Arizona, Kansas, Louisiana, Georgia, or Texas • Current BLS for Healthcare Providers certification
