Description

• Provide ongoing, family-centered case management to children, youth, and caregivers • Engage families using trauma-informed, strengths-based, and family-centered practices • Complete ongoing assessments of family needs, strengths, safety concerns, protective factors, and service barriers • Develop, implement, and monitor individualized service plans • Conduct required monthly in-home visits and additional contact based on family needs and risk • Monitor child safety, family stability, service participation, and progress toward goals • Report emerging concerns through required channels • Help families access behavioral health, medical, educational, substance use, parenting, housing, employment, transportation, and community supports • Provide practical support, coaching, and resource navigation • Communicate regularly with referring agency caseworkers and supervisors • Coordinate with schools, behavioral health providers, medical professionals, courts, juvenile probation, community organizations, and other involved systems • Participate in staffings, multidisciplinary case reviews, supervisory meetings, court proceedings, and required meetings • Coordinate and track referrals and support continuity of care • Complete case notes, assessments, service plans, contact records, court reports, weekly reports, statistical reports, and other documentation • Maintain confidential records in VisionQuest and approved systems • Follow agency procedures, state regulations, child protective services laws, mandated reporting requirements, policies, and contractual expectations • Communicate suspected abuse, neglect, safety threats, critical incidents, or significant family changes according to protocol • Participate in supervision, case consultation, quality assurance, training, and continuous quality improvement • Travel throughout the assigned service area for home visits, meetings, court proceedings, and service coordination • Transport children, youth, or family members when authorized and necessary • Maintain a flexible schedule, including evenings or nontraditional hours, as needed • Perform other assigned responsibilities

🎯 Exigences

• Bachelor’s or Associate’s degree in the behavioral sciences, human services or social services fields preferred • 1 year of case management or child welfare experience • Child welfare and/or case management experience strongly encouraged • Minimum of one year of related work experience preferred • Must be 21 years of age or older • Flexible schedule, including evenings, weekends, and holidays • Cleared Child Abuse and Neglect and FBI criminal background checks • Valid driver’s license and eligibility to drive for program services • Strong family engagement, assessment, case management, crisis response, and service coordination skills • Ability to identify and respond appropriately to child safety, family risk, and mandated reporting concerns • Excellent written and verbal communication skills • Ability to prepare clear, timely, and objective case documentation and court reports • Ability to collaborate with families, agency personnel, courts, schools, providers, and multidisciplinary teams • Knowledge of child welfare systems, trauma-informed care, family preservation practices, and community resources • Cultural humility and ability to work effectively with diverse families • Ability to work independently in community settings while maintaining communication with supervision • Strong organizational, time-management, and computer skills • Ability to learn electronic case management systems • May lift and carry up to 10 pounds frequently • Ability to stand or walk 50% of the day and sit 50% of the day • Ability to bend, stoop, reach, and grasp as necessary
