📋 Description
• Assess patient concerns and provide telephonic and video-based clinical support • Collaborate with primary care providers to develop care plans • Conduct outbound calls and virtual visits for patient follow up • Educate members on symptoms and care processes • Utilize multiple technology platforms and complete documentation in EMR
🎯 Requirements
• Minimum one (1) year experience as an RN • Knowledge of clinical standards of care • Experience assessing and triaging patient symptoms utilizing sound clinical judgment • Must have an active, unrestricted California RN license at the time of hire • CPR certification required • Excellent administrative, organizational, and verbal communication skills.
