• Empower members by supporting their health and well-being • Assist members in finding and maintaining competitive, integrated employment • Conduct face-to-face visits and assessments of members' needs • Develop and maintain Person-Centered Plans
🎯 Requirements
• RN or B.S. in social work, nursing or related fields • 2 years experience in healthcare industry • Valid driver's license • Current Certification from the Developmental Disabilities Nurses Association (preferred) • Experience as a support coordinator or case manager (5+ years in lieu of degree) • Ability to work independently or in a team • Proficient in Microsoft Office (Word, Excel, Outlook, PowerPoint)
