📋 Description

• Perform telephonic outreach to members supporting HEDIS and Stars measure gap closure and re-engagement • Provide evidence-based patient education about engagement and recommended health screenings • Use a trauma-informed, member-centric approach • Help members move through the continuum of care based on clinical or medical need • Refer members to community partners as appropriate • Complete chart reviews for regulatory and quality improvement projects • Abstract charts using EMR systems or PDF files and enter data into HEDIS or other specialized software • Maintain a 95% accuracy rate in abstractions and data entry through peer over-reads • Collaborate professionally with CareOregon clinics, team members, and staff • Complete required annual and ongoing HEDIS training and testing • Participate in other assigned projects • Perform work aligned with CareOregon’s mission, vision, values, equity, diversity, inclusion, compliance, and strategic goals

🎯 Requirements

• Current unrestricted license as a Registered Nurse in the State of Oregon • Minimum 3 years’ experience as an RN in the healthcare industry • Knowledge of or willingness to learn current clinical practices, chronic diseases, health-screening guidelines, and health maintenance • Ability to use a trauma-informed approach • Ability to extract clinical information from medical records and accurately document it in HEDIS or specialized software • Critical thinking skills • Strong attention to detail • Competency with PDF, MS Outlook, Word, Excel, and EMR systems • Ability to learn and adapt to new EMR systems, software changes, specification changes, and deadlines • Ability to maintain strict confidentiality of protected health information • Awareness of social determinants of health • Ability to manage multiple tasks, prioritize work, switch tasks, collaborate, and meet regulatory deadlines • Professional written and verbal communication skills • Ability to work effectively in a remote environment • Ability to learn, focus, understand, evaluate information, and determine appropriate actions • Ability to accept direction and feedback and manage stress • Ability to perform required visual, repetitive hand and wrist, sitting, standing, speaking, bending, lifting, carrying, pushing, pulling, and pinching activities • Preferred: Bachelor’s degree in Nursing • Preferred: Knowledge or understanding of HEDIS and previous chart abstraction experience • Preferred: Primary care or specialty clinic practice experience • Preferred: RN experience with quality improvement, population health, or case management
