Charles Lea Center is a nonprofit organization founded in 1965 in Spartanburg, South Carolina that empowers and supports people with disabilities through individualized, community-based services. The Center provides supportive living, work programs, case management, day programs, and other innovative services across South Carolina, North Carolina, and Tennessee, serving more than 3,000 people and employing over 700 staff. It operates a foundation, accepts donations, and focuses on helping clients live active, involved, and meaningful lives.

📋 Description

• Lead a team of up to 18+ Innovation Coordinators to deliver coordinated, person-centered services. • Coach and support Innovation Coordinators through regular meetings, training, and performance management. • Provide technical assistance to providers implementing services for referred individuals. • Advocate for individuals and guardians to ensure services reflect their desires. • Oversee person-centered planning and coordinated services, including transitions and provider collaboration. • Conduct 30-, 60-, and 90-day follow-ups after transition to community services. • Provide training to supervised Innovation Coordinators, people served, guardians, and community providers. • Travel to and from assigned counties in Tennessee as necessary. • Audit case files and monitor services to ensure compliance and quality. • Follow standards and regulations required by the Tennessee Department of Disability and Aging (DDA).

🎯 Requirements

• At least 18 years of age • Ability to effectively read, write, and communicate verbally in English • Ability to pass a background check upon hire • Valid driver's license • Vehicle insurance • Master's degree from an accredited college or university plus experience delivering or coordinating services and providing case management for individuals with disabilities and special needs, OR • Bachelor's degree in human services or a related field plus one year of relevant experience and proven supervisory or coaching experience, OR • Associate's degree, high school diploma, or GED plus four years of relevant experience and proven supervisory or coaching experience • Relevant experience working directly with persons with intellectual, developmental, or other disabilities or mental illness • Ability to perform regular standing, walking, lifting up to 25 lbs, and hands-on support • Willingness/ability to travel to assigned counties in Tennessee as necessary
