📋 Description

• Serve as the first and main point of contact for cancer patients and caregivers • Conduct comprehensive clinical and needs assessments • Collaborate with oncologists, primary care providers, survivorship providers, genetic counselors, care advocates, and other support team members • Provide compassionate, empathetic, patient-advocacy-focused care • Address symptom management, care escalations, nutritional support, provider referrals, and in-person care appointment planning • Communicate with patients by phone, asynchronous messaging, and video • Collaborate with Care Advocates on non-clinical support and care needs • Help develop clinical protocols and best practices • Participate in care team sessions to monitor member progress, provide updates, and collaborate on targeted support plans • Maintain thorough documentation of patient encounters • Help design Color’s oncology clinic and improve patient outcomes

🎯 Requirements

• Bachelor of Science in Nursing (BSN) required • Unrestricted active Registered Nurse (RN) license with eligibility to obtain additional state licenses as needed • Active Nurse Licensure Compact (NLC) multi-state license required • Minimum of 7 years of nursing experience, including at least 5 years in oncology healthcare settings • Outpatient experience required; inpatient experience a plus • Oncology-related certification required: Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM) • Strong organizational skills and ability to multitask in fast-paced environments • Strong verbal and written communication skills, empathy, and active listening • Comfort with video chatting, electronic health records, and telehealth tools • Medical proficiency in English and Spanish preferred • Unrestricted right to work in the United States for any employer • No company work authorization or sponsorship assistance required now or in the future
