📋 Description

• Patient Outreach & Nutritional Counseling: Manage an assigned panel of 300-350 patients and conduct required monthly nutritional outreach and support; Conduct 50-60+ daily phone calls with patients, providing nutritional care plan support and health coaching tailored to their specific conditions; Maintain call quality standards and professional communication in every patient interaction; pass regular QA call reviews. • Nutrition Planning & Patient Education: Educate patients on nutrition concepts, dietary modifications, and weight management strategies; Simplify complex nutritional information for diverse patient populations; Reinforce provider-directed nutritional guidance and support patient adherence to established care plans. • Care Coordination & Collaboration: Collaborate with clinical teams, providers, and caregivers to identify and address nutritional and clinical needs; Support care coordination goals by staying on schedule with monthly outreach targets; Escalate patient concerns or changes in condition to appropriate clinical staff per established protocols. • Documentation & Compliance: Document all patient interactions in real time using the care coordination platform; Maintain HIPAA compliance in all patient communications and documentation; Ensure accurate documentation to support care coordination goals.

🎯 Requirements

• Minimum 2+ years of experience in a healthcare role such as care coordination, health coaching, Medical Assistant • Must hold a nationally recognized medical assistant certification (CMA, RMA, CCMA, NCMA, CMAC, or equivalent state-issued MA-C) OR Active Licensed Practical Nurse or Licensed Vocational Nurse license: Must have a multi-state compact license • Minimum 2+ years of clinical nutrition counseling or dietary care coordination experience • 2+ years of Electronic Medical Records (EMR) and computer navigation experience • Demonstrated ability to educate patients and simplify complex nutritional concepts. • HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload internet capability • Must be legally authorized to work in the United States.
