📋 Description

• Assess, plan, facilitate, coordinate, evaluate, and advocate for care options and services • Meet individuals’ and families’ comprehensive health needs through communication and available resources • Promote quality, cost-effective outcomes • Perform in-person case management activities within a designated geographic area when directed or required by business needs • Work collaboratively with members, families, and available care resources • Navigate multiple systems and applications simultaneously • Communicate through email, calendar invites, Teams messaging, and virtual meetings • Access secure systems such as VPN and EHR portals • Troubleshoot common technical issues and contact IT when needed

🎯 Requirements

• Active and unrestricted Compact license, Massachusetts • Registered Nurse with active state license in good standing within the region where job duties are performed • Minimum 3–5 years of clinical practical experience preferred in areas such as diabetes, CHF, CKD, post-acute care, hospice, palliative care, or cardiac care with Medicare members • Minimum 2–3 years of case management, discharge planning, and/or home health care coordination experience • Ability to occasionally travel within a designated geographic area for in-person case management activities • Bilingual preferred • Excellent analytical and problem-solving skills • Effective communication, organizational, and interpersonal skills • Ability to work independently • Proficiency with MS Word, Excel, Outlook, PowerPoint, and proprietary applications • Efficient computer skills, including navigating multiple systems and keyboarding • Willingness and ability to obtain multistate RN licenses if needed; company will provide • Bachelor’s degree or equivalent experience; bachelor’s degree must be obtained within 3–5 years as part of role development • Certified Case Manager preferred • Additional national professional certification (CRC, CDMS, CRRN, COHN, or CCM) preferred but not required • Direct/hardwired internet connection to a modem/router within 7 feet of the computer • Minimum internet speed of 25 Mbps download and 3 Mbps upload • Quiet, secure, private, designated home workspace compliant with CVS Health and HIPAA guidelines • Must work within the state and city where currently living • Ability to use multiple monitors and applications simultaneously • Ability to use Microsoft Office 365 applications or similar tools such as Google Workspace • Ability to log into secure systems, use VPN and EHR portals, lock screens, manage strong passwords, and recognize suspicious emails or links • Ability to resolve common technical issues independently • Openness to learning new workplace technology skills
