• Review consultant prescribing recommendations and perform full clinical pharmacy checks before issuing prescriptions • Check allergies, dose, frequency, formulation, interactions, clinical appropriateness and local ICB formulary compliance • Issue NHS and private patient prescriptions using remote prescribing software • Apply local ICB formularies and prescribing restrictions across multiple Integrated Care Boards • Provide feedback to consultants and make safe prescription changes within scope where appropriate • Counsel patients by telephone on medicines, side effects, monitoring and when to seek advice • Liaise with consultants and GPs regarding prescription clarification, amendment or escalation • Manage specialist and high-risk medicines under shared care and governance requirements • Identify and escalate medicines safety concerns, interactions and contraindications • Maintain accurate records of clinical interventions and patient contacts • Support the Lead Pharmacist with medicines governance, audits, quality improvement and pathway development
• MPharm degree and current GPhC registration • Active Independent Prescriber qualification at the point of engagement • Experience in a secondary care, outpatient or hospital pharmacy setting • Membership with PDA or equivalent professional indemnity in place • Minimum 2 years post-registration clinical experience with active Independent Prescriber practice • Proficient in remote clinical and prescribing systems • Ability to work effectively in a fully remote environment • Strong verbal and written communication skills • Private and confidential space to work at home • Current UK professional GPhC registration • Subject to a Disclosure and Barring Service (DBS) check • Desirable: experience in a remote or virtual clinical environment • Desirable: postgraduate certificate or diploma in pharmacy practice (PGCert or PGDip) • Desirable: familiarity with local ICB formularies and shared care protocols • Desirable: involvement in medicines governance, SOP development or clinical audit
