📋 Description

• Provide synchronous and asynchronous telemedicine consultations for older adults • Conduct virtual assessments and review medical history • Identify health-related social needs (HRSNs) as part of the care planning process • Create and update care plans that guide Community Care Advocates in providing community health support services • Participate in asynchronous chart reviews and escalations as needed • Collaborate with Papa Docs’ partners and operational teams to inform workflows and implement care plans • Integrate medical and social care delivery to improve overall well-being of Medicare beneficiaries

🎯 Requirements

• Active, unrestricted MD or DO license in good standing • Board-certification in internal Medicine or Family Medicine • Active Florida state medical license, Florida Medicaid credential and Medicare enrollment required • Licensure or credentialing in additional states is a plus • Minimum 2 years post-residency clinical experience • Prior experience with telemedicine strongly preferred • Flexibility to work a minimum of 8 hours/week for synchronous consultations on weekdays • Excellent communication and clinical reasoning skills • Compassionate, patient-first mindset • Comfort working in a fast-moving, tech-enabled environment (experience with telemedicine is a plus) • Flexibility, reliability, and collaboration in cross-functional teams • Fluency in Spanish is a bonus
